Please note we do not charge an intake fee for any assessment.  One appointment and you will receive your assessment, recommendations, and referrals.

ASSESSMENTS
 

State-Certified ADSAC Alcohol / Drug Assessments

Required for Drivers License Reinstatement / Court Ordered / D.A. Supervision. 
Cost - If the arrest occurred after November 1st, 2008 - the fee is $160.00.  Before November 1, 2008 - the fee is $175.  Assessment time is approximately 1½ - 2 hours.

Substance Abuse Assessments for Supervised (PO) Probation / Parole

This Assessment is ordered by your Probation Officer and is NOT for Drivers’ License Reinstatement. A letter of recommendation will be sent to your Probation Officer.  Please bring your PO name and fax number to your scheduled appointment.  The fee is $160.00. Assessment time is approximately 1 - 2 hours.

Work-Related Referral for Substance Abuse Assessments

Required by Employer. The fee is $160.00. Assessment time is approximately 1 - 2 hours.
